Indication:

Pregabalin is an anti-convulsant medication used for the prevention of neuropathic pain, epilepsy, fibromyalgia, and neuralgia. It works by calming damaged or overactive nerves and reducing the number of pain signals sent by damaged nerves. Potential side effects include dizziness, somnolence, dry mouth, edema, blurred vision, weight gain, and abnormal thinking. Inform your doctor about symptoms of suicidal ideation, drug addiction, pulmonary issues, muscle pain, weakness, and kidney, liver, or heart diseases. Caution is advised for pregnant or breastfeeding women, and the safety of children under 18 years is not established.

Pharmacology:

Pregabalin is a structural derivative of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA). It does not have a specific binding affinity for GABAA, GABAB, or benzodiazepine receptors. Instead, pregabalin exhibits high affinity to the alpha-2-delta subunit, which is an auxiliary subunit of voltage-gated calcium channels, particularly in tissues of the central nervous system.

The oral bioavailability of pregabalin is equal to or greater than 90%, and it is not significantly affected by dosage. The elimination of pregabalin from the systemic circulation primarily occurs through renal excretion in the form of an unchanged drug. The elimination half-life of pregabalin is approximately 6.3 hours in individuals with normal renal function.

Dosage & Administration of Neugalin XR 165mg Capsule

Adult Dose:

Neuropathic pain, Postherpetic Neuralgia:

• Initially: 150 mg/day
• May increase to 300 mg/day after 3-7 days.
• Maximum: 600 mg/day after a 7-day interval.
• All doses to be given in 2 or 3 divided doses.

Diabetic Peripheral Neuropathic Pain:

• Initial: 50 mg PO q8hr
• Maintenance: May increase to 100 mg PO q8hr within 1 week, as needed.
• Not to exceed 300 mg/day

Adjunct in partial seizures:

• Initially: 150 mg/day
• May increase to 300 mg/day after a week.
• Maximum: 600 mg/day.
• All doses to be given in 2 or 3 divided doses.

Fibromyalgia:

• Initially: 150 mg/day
• May increase to 300 mg/day after a week.
• Maximum: 450 mg/day, if needed.
• All doses to be given in 2 or 3 divided doses.

Child Dose: Safety and efficacy not established.

Renal Dose for Renal Impairment:

Hemodialysis:

• 25-100 mg immediately after each 4-hr hemodialysis session.

CrCl (ml/min) Dosage Recommendation:

• 30 to <60: 75 mg/day. Max: 300 mg/day. All doses to be given in 2 or 3 divided doses.
• 15 to <30: Initially, 25-50 mg/day. Max: 150 mg/day. All doses are to be given as a single dose or in 2 divided doses.
• <15: Initially, 25 mg/day. Max: 75 mg/day. All doses are to be given as a single dose.

Interaction

Pregnancy & Lactation

Pregabalin, like other medicines, can pass into breast milk; thus, it should only be used in nursing mothers if the benefits clearly outweigh the risks

Precautions & Warnings

Talk to your doctor if:

• You develop an allergic reaction such as bumps, itching, swelling around your face and other body parts, or difficulty breathing after taking the Pregaba Capsule. Seek medical help immediately.
• You have a heart problem, diabetes, or kidney function problems.
• You are above 60 years of age or have any respiratory disorders. Pregaba Capsule can cause dizziness, leading to accidental injury (falls) in elderly patients. Be cautious if you are an elderly patient or giving this medicine to an elderly patient.
• You are experiencing vision problems, dizziness, sleepiness, stomach discomfort, constipation, etc.
• If you experience suicidal thoughts; talk to your doctor immediately.
• You have a history of fits.
• Do not stop this medicine abruptly; your doctor shall gradually decrease the dose.
• Pregaba Capsule is not recommended in patients below 18 years of age.
